Apollo Homecare, a leading homecare service provider in India, has successfully launched its innovative ‘Home Discharge Program’ aimed at prioritising post-discharge care for cardiac surgery patients. The company in a press statement on Tuesday said that this initiative signifies a significant step towards enhancing recovery and ensuring the long-term well-being of patients undergoing cardiac procedures.
Vishal Lathwal, CEO of Apollo Homecare, highlighted the growing prevalence of cardiac diseases in India due to sedentary lifestyles and unhealthy habits. He emphasised the importance of post-discharge care in expediting recovery and ensuring patients receive quality care within their homes.
The program focuses on real-time patient monitoring, providing access to expert doctors, physiotherapists, and trained nurses to facilitate continuous care at home. This specialised approach addresses the critical period following discharge, reducing the risk of complications and promoting a seamless transition from hospital to home.
Dr. Sai Kumar of Apollo Homecare commended the success of the program, citing positive outcomes such as reduced readmission rates and increased compliance with follow-up appointments. The use of risk-stratification tools has further improved patient outcomes by identifying those at high risk and enabling targeted interventions.
The initial phase of the program, launched in Hyderabad, witnessed over 900 enrollments with a readmission rate of 3 in 100 among enrolled patients.
