How Moses 2.0 Technology Is Revolutionising Urologic Treatments

There are various lasers available in the market which is widely used in medical practice especially in the field of urology and ophthalmology, and less frequently in dentistry and other specialities. Its use in the treatment of kidney stones and prostate enlargement (BPH)) has taken a steep rise in the last 5 years. 

Currently, the lasers used in urology practice are low-watt holmium, high-power holmium, Thulium, Thallium fibre lasers (TFL), Diode and KTP (potassium titanyl phosphate). Each has specific roles to play in the management of kidney stones, BPH (prostate enlargement) and certain cancers of the bladder and kidney.

All lasers work on the principle of a certain wavelength with tissue penetration capability, just enough to target and cut the tissue with minimal or no bleeding. These lasers have the capability to vaporize the tumour in question or fragment the stones when targeted. 

For this, one requires a target lesion and a medium for the laser to pass through while targeting. In urology practice, the target organs are the kidney stones, prostate and certain tumors, and endoscopes being the medium for passing the laser fibers.

Among the various lasers available, Holmium is the most commonly used worldwide, is very effective, and can be used widely to treat urological problems like kidney stones, BPH and bladder tumors. They are available as low power and high power (30 watt, 100watt &120 watt) lasers. Recently, pulse modulated laser like "Moses" (high power holmium) has revolutionized the way kidney stones and prostate are treated. 

This means that any size of the kidney stone can be dusted (which is not seen in most other lasers) and any size of the prostate with any co morbidity can be completely removed with the least hospital stay.

The Moses laser (p120h Boston Scientific): This is a new innovative high power (P 120) holmium laser used specifically for kidney stones and BPH treatment. This laser works with the principle of plasma bubble in bubble technology through pulse modulation. The bubble helps in the microdissection of the tissue to be removed effectively and fragment/dust the stone in question. This is possible because the physics of the holmium laser has been re-framed to suit the treatment.

The Moses laser has a wide application in terms of clinical practice, especially in the treatment of kidney stones and the prostate gland. This is due to the laser’s ability to dust the stones inside the kidney, irrespective of the size and hardness (composition of the stone), and to resect (remove) the prostate irrespective of the size and complexity. This Moses effect when applied for prostate treatment is called Molep (Moses laser enucleation of the prostate ), and when it is applied for kidney stones it is called MoTSF( Moses technology stone fragmentation and dusting).

The advantages of Holmium laser with Moses effect (Moses technology) in prostate and kidney stones include lesser pain, less blood loss, minimally invasive, any size of prostrate can be treated, less hospital stay, and no scar.
